(1) Attacks of migraine without aura that occur regularly on day 1 of menstruation and last ± 2 days, occurring at no other time.
(2) A migraine that waxes/wanes in intensity with menstruation.
 
Clinical findings
PMS symptoms which occur during the luteal phase and include depression, anxiety, crying spells, difficulty thinking, lethargy, backache, breast tenderness, swelling, nausea; migraine and tension headaches may overlap.
 
Presentation
Over half of women with migraines relate the headaches’ periodicity to menstruation; 15 to 25% have attacks exclusive in the 2 to 3 days before the onset of menstruation. Thus defined, 7% of female migraineurs have only menstrual migraine; menstruation is a trigger for about 60% of migraineurs.
 
Management
Percutaneous estradiol may control the headaches in women with dysmenorrhoea or ovarian dysfunction.

Q. preventing pre menstrual MIGRAINES. Has anyone come up with any good preventive medicine for migraines? Mine come systematically right before and during the menstrual cycle. My (male) neurolight in Paris didn't seem to see a linkk with migraines and my cycle - or offer any preventive advice. I take Relpax to relieve. Does anyone have a more natural or preventive solution? A. I have the same problem and take Topomax to prevent the migrains. Also make sure you get enough rest right before you start. There seems to be a correlation. It took about 2 months to fully work but at least the first migrain after I started taking Topomax wasn't as severe.
