Form 5

A form that an executive or other insider must file with the SEC reporting and disclosing all previously undisclosed, legal insider trading that he/she conducted over the previous fiscal year. Form 5 is especially used for small insider trading transactions that are not required to be disclosed under Form 4. Form 5 exists to promote transparency in the market.
