The SIP consists of 136 statements about limitations in 12 categories, with either physical and psychosocial dimensions.

A standardized questionnaire for assessing the effects of illness on function. There are 136 questions grouped into 12 categories. The categories include sleep and rest, eating, work, home management, recreation, ambulatory mobility, body care, movement, social interaction, alertness, emotional behavior, and communication.
