Acute Pulmonary Coccidioidomycosis

Lung infection due to Coccidioides immitis or C posadasii, which is endemic in southwestern US.
Epidemiology 10- to 30-day incubation period; more common in immune compromised hosts.
Clinical findings 60% remain asymptomatic; some may develop chest pain of varying intensity, chills, cough, fever, head and muscle aches, musculoskeletal stiffness, lymphadenopathy, night sweats, rash, erythema nodosum, weight loss.
Management Usually self-limited; if therapy is required, amphotericin B, fluconazole, itraconazole.
