misconnection error

The linking of two sets of wires or tubes that share a common fitting and therefore seem to work with each other even though they may carry incompatible or dangerous substances that should not be administered to a patient.

Patient care

Misconnection errors can be prevented by carefully tracing each tube to its source and verifying its contents; or systematically by manufacturing unique fittings or connectors that cannot link incompatible agents.
