A term which, as defined in the UK, refers to an illness which lasts longer than a year, often worsens with time, causes limitations to the patient’s physical, mental and/or social well-being, and requires infrastructural and social support to allow sufferers to live independently, by providing transport, housing, employment, education, benefits and pensions. Multiple long-term conditions make care particularly complex, and a small number of individuals with long-term conditions account for a disproportionate use of healthcare, especially hospital care. 60% of adults in England report a chronic health problem; 8.8 million in the UK have long-term condition which severely limits their daily lives.
