Occlusive renal arterial disease with stenosis and ischemia, resulting in ↑ renin, HTN and azotemia, affecting one or both kidneys,.2. ↓ Effective circulatory volume, due to ↑ vascular capacitance–eg, induced by sepsis, sequestration of fluids in compartments–eg, in the hepatorenal syndrome or ascites, or the inability to effectively transfer fluids from the venous compartment to the arterial compartment due to CHF or pericardial limitations due to constrictive pericarditis or effusions; the ↓ volume form of hypoperfusion is associated with oliguria, azotemia, ↓ sodium excretion and ↑ renin, but not HTN.
