Etymology: < classical Latin quadriformis compounded of four figures, in post-classical Latin also having four forms (4th cent., translating Hellenistic Greek τετράμορϕος : see tetramorph n.) < quadri- quadri- comb. form + -formis -form comb. form2. Compare Italian quadriforme (1595).
