Origin: A borrowing from Latin. Etymon: Latin quaestionarius.
Etymology: < post-classical Latin quaestionarius, questionarius (in discourse) questioner (a805 in a British source; first half of the 14th cent. in the passage translated in quot. 1435 at sense 1), person who carries out a judicial enquiry (from 12th cent. in British sources), alms collector, pardoner (13th cent. in British sources; this sense probably influenced by quaestor , questor quaestor n.) < classical Latin quaestiōn- , quaestiō question n. + -ārius -ary suffix1. Compare classical Latin quaestiōnārius, used as a military title (perhaps ‘interrogator’).

Origin: Formed within English, by derivation; partly modelled on a French lexical item, and perhaps partly modelled on a Latin lexical item. Etymons: question n., -ary suffix1.
Etymology: < question n. + -ary suffix1, in sense 1 after Middle French questionnaire, questionaire (1533 in the title of Le questionnaire des cirurgiens: compare quot. ?1541) or its etymon post-classical Latin quaestionarius, questionarius catechism, handbook (10th cent.; also as quaestionarium (14th cent.)), in sense 2 after French questionnaire (1846). With sense 2 compare later questionnaire n. and Italian questionario (1882; after French). Compare earlier questionary n.1
